Abstract

Komunikasi terapeutik berperan penting dalam pelayanan keperawatan di ruang Intensive Care yang memiliki karakteristik kondisi pasien kritis, tekanan kerja tinggi, dan kebutuhan informasi keluarga yang kompleks. Situasi tersebut kerap memunculkan hambatan komunikasi yang dapat memengaruhi efektivitas pelayanan kualitas pertukaran informasi, serta hubungan perawat–keluarga. Penelitian ini bertujuan menggambarkan hambatan komunikasi terapeutik antara perawat dan keluarga pasien di ruang ICU dan NICU RSUD Umar Wirahadikusumah. Metode penelitian menggunakan kuantitatif dengan desain deskriptif. Populasi terdiri atas seluruh perawat ICU dan NICU berjumlah 52 orang dan seluruhnya dijadikan sampel melalui teknik total sampling. Instrumen penelitian menggunakan kuesioner Barriers to Nurse-Patient Communication Questionnaire yang telah dimodifikasi sesuai konteks ruang intensive care. Analisis data dilakukan secara deskriptif untuk memperoleh distribusi frekuensi dan persentase hambatan komunikasi terapeutik. Hasil penelitian menunjukkan bahwa sebanyak 48 perawat (92,3%) berada pada kategori hambatan sedang, sedangkan 4 perawat (7,7%) berada pada kategori rendah. Hambatan yang paling menghambat adalah hambatan individual-sosial. Temuan ini mengindikasikan bahwa interaksi perawat dan keluarga masih dipengaruhi oleh kondisi kerja intensive care yang dinamis, beban tugas yang tinggi, serta perbedaan pemahaman informasi kesehatan. Berdasarkan hasil tersebut, diperlukan penguatan keterampilan komunikasi, dukungan lingkungan kerja yang lebih kondusif, serta penyusunan SOP komunikasi sebagai pedoman bagi perawat dalam memberikan informasi serta meningkatkan efektivitas hubungan perawat dan keluarga. Therapeutic communication plays a crucial role in nursing services in the Intensive Care Unit, characterized by critical patient conditions, high work pressure, and complex family information needs. These situations often create communication barriers that can impact the effectiveness of service, the quality of information exchange, and the nurse-family relationship. This study aims to describe the barriers to therapeutic communication between nurses and patient families in the ICU and NICU of Umar Wirahadikusumah Regional Hospital. The research method used a quantitative approach with a descriptive design. The population consisted of all 52 ICU and NICU nurses, all of whom were sampled using a total sampling technique. The research instrument used the Barriers to Nurse-Patient Communication Questionnaire, modified to suit the context of the intensive care unit. Data analysis was conducted descriptively to obtain the frequency distribution and percentage of barriers to therapeutic communication. The results showed that 48 nurses (92.3%) were in the moderate barrier category, while 4 nurses (7.7%) were in the low barrier category. The most significant barrier was individual-social barriers. These findings indicate that nurse-family interactions are still influenced by the dynamic working conditions of intensive care, high workloads, and differences in understanding of health information. Based on these results, it is necessary to strengthen communication skills, support a more conducive work environment, and prepare communication SOPs as a guideline for nurses in providing information and increasing the effectiveness of the relationship between nurses and families.
