Pharmacology subject: RT Nursing description: Latar Belakang: Congestive Heart Failure (CHF) merupakan salah satu masalah kesehatan utama yang berkontribusi terhadap tingginya angka morbiditas dan mortalitas. Gejala dispnea yang dialami pasien CHF sering kali tetap muncul meskipun terapi farmakologis telah diberikan secara optimal. Kondisi ini menimbulkan hambatan dalam melakukan aktivitas sehari-hari serta menurunkan kualitas hidup pasien. Oleh karena itu, diperlukan intervensi non-farmakologis yang dapat digunakan sebagai terapi pendukung. Salah satu intervensi yang potensial adalah Hand Held Fan (HHF), yaitu terapi yang menggunakan kipas genggam kecil. Terapi ini bekerja melalui stimulasi saraf trigeminal pada wajah sehingga mampu mengatasi dispnea. Tujuan: Mengetahui penerapan terapi HHF untuk mengatasi dispnea pada pasien CHF. Metode: Penelitian ini menggunakan desain studi kasus tunggal pada pasien CHF berusia 60 tahun yang dirawat di ruang rawat inap penyakit dalam. Intervensi dilakukan dengan memberikan terapi HHF selama 5 menit setiap sesi, dengan frekuensi 1 kali per hari selama 3 hari berturut-turut. Data dikumpulkan melalui pengukuran laju napas (RR), saturasi oksigen (SpO2), serta skala dispnea menggunakan Modified Borg Scale (MBS) sebelum dan sesudah intervensi. Hasil: Hasil penelitian menunjukkan adanya perbaikan klinis setelah pemberian terapi HHF. Laju napas pasien menurun dari 24 x/menit menjadi 20 x/menit, saturasi oksigen meningkat dari 93% menjadi 97%, serta skor dispnea pada MBS menurun dari 2 menjadi 0. Selain itu, pasien melaporkan napas terasa lebih lega, lebih nyaman, dan toleransi terhadap aktivitas sehari-hari meningkat. Kesimpulan: Terapi HHF efektif digunakan sebagai intervensi non-farmakologis untuk mengatasi dispnea pada pasien CHF. Intervensi ini dapat direkomendasikan sebagai bagian dari pendekatan keperawatan holistik untuk meningkatkan kenyamanan dan kualitas hidup pasien. Kata kunci: Congestive Heart Failure, Dispnea, Hand Held Fan Background: Congestive Heart Failure is one of the major health problems that contributes to high morbidity and mortality rates. Dyspnea experienced by patients with CHF often persists despite the administration of optimal pharmacological therapy. This condition creates limitations in performing daily activities and reduces the patient’s quality of life. Therefore, non-pharmacological interventions are needed as supportive therapy. One potential intervention is the use of a hand held fan, a simple therapy that employs a small portable fan. This intervention works by stimulating the trigeminal nerve on the face, thereby helping to relieve dyspnea. Objective: To determine the application of hand held fan therapy in reducing dyspnea among patients with Congestive Heart Failure. Method: This study employed a single case study design involving a 60-year-old patient with CHF who was hospitalized in the internal medicine ward. The intervention was carried out by providing hand-held fan therapy for 5 minutes per session, once daily, for three consecutive days. Data were collected through measurements of respiratory rate (RR), oxygen saturation (SpO2), and dyspnea score using the Modified Borg Scale (MBS) before and after the intervention. Result: The findings showed clinical improvement following the administration of hand-held fan therapy. The patient’s respiratory rate decreased from 24 breaths/minute to 20 breaths/minute, oxygen saturation increased from 93% to 97%, and the dyspnea score on the MBS decreased from 2 to 0. In addition, the patient reported feeling easier to breathe, more comfortable, and demonstrated improved tolerance to daily activities. Conclusion: Hand-held fan therapy is effective as a non-pharmacological intervention to reduce dyspnea in patients with Congestive Heart Failure. This intervention can be recommended as part of a holistic nursing approach to enhance patient comfort and quality of life.
