    Abstract

The construction industry is one of the sectors with a high risk of occupational accidents, especially in high-rise building projects. This study aims to analyze the risk of occupational accidents in educational building construction projects in Sumedang Regency, West Java, with a focus on identifying the dominant risk factors and control measures. This study used a questionnaire survey method with 10 respondents who were directly involved in the project with at least two years of experience. The analysis was conducted using a likelihood and severity matrix based on the AS/NZS 4360:2004 standard, as well as Likelihood Index and Severity Index calculations to determine the risk level classification. Furthermore, the Bowtie method was used to illustrate the relationship between causes, events, and risk impacts, as well as to formulate prevention and mitigation controls. The results of the study show that there are 45 risk factors, with four dominant high-risk factors: (1) workers falling from heights during scaffolding installation, (2) workers being cut/slashed due to the use of sharp tools in ironwork, (3) workers falling from heights during casting, and (4) workers falling during formwork dismantling. Bowtie analysis shows that the main causes of risk are unsafe working conditions and worker negligence, with the impact of serious injury to death. Recommended control measures include the use of standard PPE, installation of fall arrest systems, and increased training and routine OSH supervision. This study emphasizes the importance of Bowtie-based risk management as a strategy to minimize workplace accidents and improve safety in construction projects.
