Abstract

Bronkopneumonia merupakan infeksi saluran napas bawah yang sering terjadi pada anak dan dapat menyebabkan gangguan bersihan jalan napas. Salah satu intervensi nonfarmakologis yang digunakan adalah fisioterapi dada berupa postural drainage dan clapping. Tujuan laporan kasus ini adalah untuk mengevaluasi efektivitas postural drainage dalam mengatasi masalah bersihan jalan napas tidak efektif pada anak dengan bronkopneumonia. Penulisan menggunakan metode studi kasus deskriptif kualitatif pada pasien anak perempuan usia 19 bulan yang dirawat di ruang rawat inap anak RSUD Umar Wirahadikusumah. Data dikumpulkan melalui observasi langsung, wawancara keluarga, pemeriksaan fisik, serta dokumentasi asuhan keperawatan, lalu dianalisis secara naratif. Intervensi yang diberikan meliputi manajemen jalan napas, terapi nebulisasi, dan fisioterapi dada (postural drainage, clapping, vibrasi) yang dilakukan dua kali sehari selama tiga hari. Hasil menunjukkan adanya penurunan frekuensi napas dari 45 menjadi 30 kali/menit, peningkatan saturasi oksigen dari 98% dengan oksigen menjadi 99% tanpa oksigen, serta menghilangnya suara napas tambahan dan pengeluaran sekret menjadi lebih mudah. Selain itu, anak tampak lebih nyaman, aktif, dan tidur nyenyak. Evaluasi menggunakan pendekatan SOAP menunjukkan bahwa masalah bersihan jalan napas tidak efektif dapat teratasi sepenuhnya pada hari ketiga intervensi. Dapat disimpulkan bahwa postural drainage efektif sebagai intervensi keperawatan dalam meningkatkan bersihan jalan napas pada anak dengan bronkopneumonia. Bronchopneumonia is a common lower respiratory tract infection in children and can cause impaired airway clearance. One non-pharmacological intervention used is chest physiotherapy in the form of postural drainage and clapping. The purpose of this case report is to evaluate the effectiveness of postural drainage in addressing the problem of ineffective airway clearance in children with bronchopneumonia. This case study used a qualitative descriptive case study method in a 19-month-old girl hospitalized in the pediatric inpatient ward of Umar Wirahadikusumah Regional General Hospital. Data were collected through direct observation, family interviews, physical examinations, and nursing care documentation, then analyzed narratively. Interventions provided included airway management, nebulization therapy, and chest physiotherapy (postural drainage, clapping, vibration) performed twice daily for three days. Results showed a decrease in respiratory rate from 45 to 30 breaths/minute, an increase in oxygen saturation from 98% with oxygen to 99% without oxygen, the disappearance of adventitious breath sounds and easier expectoration of secretions. In addition, the child appeared more comfortable, active, and slept soundly. Evaluation using the SOAP approach showed that the problem of ineffective airway clearance was completely resolved on the third day of intervention. It can be concluded that postural drainage is an effective nursing intervention in improving airway clearance in children with bronchopneumonia.
